Case Management

The case management department at Stevens is available to help patients and family members with healthcare assistance during and after their stay at Stevens. Case Management nurses and social workers coordinate patient hospital care, assist patients and families with insurance issues and discharge planning, facilitate family healthcare decisions, and provide grief assistance. Additionally, they make referrals to community services for inpatients who need continuing care post hospital stay, including home health services, mental health support, and substance or physical abuse support.

The Emergency Department has social workers who assist in community referrals and intake evaluations for mental health issues, including psychological assessment, substance abuse and physical abuse.
